Community Health Care is excited to offer you a new care management program, ChartSpan. Care management is designed for individuals with Medicare, connecting you with a personal care coordinator who knows you and your needs.

ChartSpan also provides:

  • 24/7 care line where you can reach clinicians and nurses
  • Guidance on setting health goals and creating a customized care plan
  • Help with finding community resources, like transportation
  • Assistance with setting appointments and filling prescriptions
