The 2026 – 2027 school year is fast approaching and there are so many tasks that need to be done… September will be here before you know it.
Let Community Health Care help!

We will be hosting health fairs at various clinic locations throughout the summer. We are continuing to add events.
Make sure you check back here regularly to see if there will be a health care event at a Community Health Care clinic near you. Once you find the most convenient date and location, register! If you are bringing more than one child, please fill out registration forms for each child.

If a registration form is not yet available, please check back! We are adding the registration forms as event details are finalized.

If possible, please download, print, and fill out the appropriate forms before you arrive for your appointment.
