Medical Assistant II- Integrated Care Team

Under the supervision of the Clinical Manager and in partnership with the Clinic QI specialist and Outreach Welcome Center staff member, promotes access of Managed Care members assigned to Community Health Care and active patients who meet the population for the Advanced Payment Method- 4 (APM-4), ensures that clinical measures are met, and ensures that all open risks are closed at subsequent visits. This role additionally assists to organize the work of the Medical Assistant staff on their team and within their Community Health Care clinic; assists in resolving operational problems, improvement of quality outcomes, and assessment of clinical work flow; aAssists with training activities, data collection, policy and procedures implementation and work flow changes related to the Patient Centered Medical Home (PCMH) model and Value based payment systems.

Minimum Requirements:

  1. High School Diploma or equivalent
  2. Graduation from an accredited, state approved Medical Assistant Program that includes phlebotomy, medical terminology, CPT and ICD-9 coding procedures and math
  3. CPR certification
  4. At least 1-2 years previous experience working in a clinic environment or 6 months in a Residency environment.

Preferred Qualifications:

  1. Community health care and ambulatory care experience.
  2. Bilingual capacity
