Pharmacy Manager/Clinical Pharmacist

The Pharmacy Manager works under the direction of the Pharmacy Director and Medical Director and is responsible for the day to day operations in the Agency’s Pharmacy and for managing the Pharmacy staff. The Clinical Pharmacist ensures the safe and effective use of medication by providing medication order verification while assessing for optimal drug product selection, dosage regiment design, effective administration and drug therapy monitoring services at Community Health Care. Monitors patient drug therapies when appropriate and works with the appropriate medical staff to find alternatives when needed. Also facilitates medication reconciliation process and works closely within a multidisciplinary team to ensure the best care is provided. Proactive clinical interventions with providers, patients and QI to meet standard measures and goals required by Federal, State and MCO’s. Assist with Specialty Pharmacy, Hep C, etc. and partner with providers and patient’s optimal drug selection and therapeutic outcome

Minimum Requirements:
  1. Doctor of Pharmacy Degree (Pharm D)
  2. Completion of a pharmacy practice or specialty residency (PGY1 or PGY2) or hospital clinical skill sets and experience
  3. Active Pharmacist License in the State of Washington
  4. Active Preceptors License
  5. Two yrs of experience as a clinical pharmacist within involvement in formulary development, drug use evalution and quality assurance activites
  6. 1 - 3 years’ experience directly related to the duties and responsibilities specified including supervisory experience
Preferred Qualifications:
  1. Experience in community health care/ ambulatory care setting.
  2. Bilingual capacity.
  3. Knowledge of Federal 330 and private grant requirements.
