Living Well With Diabetes And Chronic Conditions

Community Health Care is now offering the Living Well with Chronic Conditions and Living Well with Diabetes workshops for our patients and our community. 

These workshops are led by two trained leaders, with topics that include; techniques to deal with difficult emotions, appropriate exercise, healthy eating, medication management, communication skills, and working effectively with health care providers. 

All adults interested, whether they are a Community Health Care patient or not, are encouraged to register if they have a chronic illness, diabetes, or pre-diabetes.

Living Well with Diabetes
Open to anyone with prediabetes, diabetes type I or type II, and caregivers. It is encouraged for participants to attend all 6 sessions.
